Job Opportunity: Palliative Care Nurse Practitioner

AlignMed Partners is seeking a dedicated and compassionate Palliative Care Nurse Practitioner to join our team. This is a full-time position (40 hours per week) with a competitive hourly range of $55-$58.

Key Responsibilities:

  • Provide primary and urgent care services to nursing center residents in collaboration with other medical providers.
  • Work collaboratively with Center Leadership to achieve AlignMed Partners' mission of delivering high-quality healthcare and everyday compassion.
  • Collaborate with the Medical Director and/or Attending Physician for patient-specific medical care.

Requirements:

  • Graduate from an accredited School of Nursing program and licensed as a registered nurse.
  • Certified as a Nurse Practitioner with a Master's Degree in Nursing and a concentration in Older Adult and/or Family Nurse Practitioner.
  • Minimum of two years' experience working with adult or geriatric populations.
  • APRN with an unrestricted professional license and DEA in states where practice allows.
