- Develop and implement effective discharge plans to ensure continuity of care.
- Provide advocacy and education to patients and families to empower them to make informed decisions.
- Respond to crisis situations and provide counseling and support to patients and families.
- Facilitate patient communication and decision-making regarding post-acute care.
- Provide social work support to outpatients as needed.
- State of Oregon Licensed Clinical Social Worker (LCSW) required.
- 1 year of experience as a Clinical Social Worker Associate or 2 years of post-MSW experience.
- 5 years of post-MSW experience preferred.
