About the Job

We are seeking experienced LPNs to join our team in Greensburg, PA, and surrounding areas. As an LPN with IntelyCare, you will have the opportunity to work in post-acute facilities, providing high-quality patient care and making a meaningful difference in the lives of our residents.

Responsibilities
  • Provide direct patient care, including administering medications, monitoring vital signs, and documenting health concerns
  • Collaborate with healthcare teams to develop and implement resident care plans
  • Conduct intakes, document health concerns, and monitor medical history of residents
  • Administer and verify medications, monitoring their effects and reporting any concerns
  • Review residents' charts and medical records, ensuring accurate and up-to-date information
  • Assist with the cultivation of a safe and comfortable facility environment
  • Maintain appropriate stocks of medical supplies and participate in IV set up, immunizations, and phlebotomy procedures
Qualifications and Skills
  • Current Pennsylvania state license to practice as a Licensed Practical Nurse (LPN)
  • Completion of approved Licensed Practical Nurse (LPN) educational program
  • H.S. diploma or GED (or equivalent)
  • Ability to lift a minimum of 50 lbs
  • Minimum of 6 months of experience (LPN or related role)
