Position Overview

Westford House Health Center, an esteemed affiliate of Best Care, is seeking a dedicated MDS Nurse. This role is pivotal in ensuring the completion and submission of precise and timely MDS Assessments, CCAs, and Care Plans in compliance with CMS RAI Manual Regulations and all relevant laws and standards.

Qualifications
  • Graduated from an accredited LPN or RN program.
  • Must hold a current and active nursing license in the applicable State, maintaining good standing throughout employment.
  • A minimum of two years of nursing experience is required, with a preference for experience in geriatric nursing.
  • Possession of CRN C Certification (clinical compliance) is necessary.
  • CPR certification is required upon hire or must be obtained during orientation, with ongoing maintenance throughout employment.
  • Experience with Point Click Care (PCC) and PDPM is essential.
Key Responsibilities
  • Conduct and submit MDS Assessments, CCAs, and Care Plans accurately and promptly in accordance with CMS RAI Manual Regulations.
  • Communicate any changes in patient conditions identified through MDS Assessments to the Director of Nursing.
  • Provide educational support to direct care staff regarding updates or changes to CMS RAI Manual or Skilled Nursing Facility Regulations that affect documentation.
  • Assist in the evaluation of the Interdisciplinary Comprehensive Care Plan.
  • Review Final Validation Reports to ensure all assessments are accepted into the CMS QIES system prior to billing, and notify the Business Office of any assessments that are not ready for billing.
  • Analyze CMS Reports to identify late submissions and develop strategies to prevent future occurrences.
  • Participate in Daily PPS Meetings, Monthly Triple Checks, and other relevant meetings as needed in the absence of the RN MDS Coordinator.
  • Fulfill the duties of a staff nurse as required.
  • Exhibit exceptional customer service and maintain a positive demeanor towards patients.
  • Assist in patient evacuation procedures when necessary.
  • Demonstrate consistent and reliable attendance.
  • Utilize reasoning skills and sound judgment in all tasks.
