The Community Engagement Nurse Coordinator plays a vital role in executing Hackensack Meridian Health's Community Outreach Wellness Strategy, aimed at addressing the health needs of the community while aligning with our strategic goals and centers of excellence. This role involves the implementation and assessment of health education initiatives and the coordination of health education services.

This position requires community travel and offers a hybrid work model (2 days in the office and 3 days in the community/home). Flexibility in scheduling is essential, as hours may vary weekly.

Key Responsibilities

A typical day for a Community Engagement Nurse Coordinator at Hackensack Meridian Health includes:

  • Executing designated outreach initiatives as per the community outreach strategy, ensuring timely and accurate completion of logistics for health screenings, fairs, educational programs, and special events.
  • Representing the Hackensack Meridian Health Community Outreach department on relevant boards, coalitions, and committees that align with our community benefit objectives. Participating in committees as directed and communicating pertinent information to the department.
  • Building and maintaining relationships with key community stakeholders committed to enhancing health within the Hackensack Meridian Health service areas, supporting the Community Health Needs Assessment Implementation Plan.
  • Coordinating nursing and health promotion services based on community assessments and identifying at-risk populations.
  • Regularly collecting and entering data into outreach databases as directed, and preparing monthly management reports summarizing outreach activities and outcomes.
  • Collaborating with nurses, health educators, community members, and healthcare professionals to schedule health-related activities and organize presentations by medical experts.
  • Gathering and disseminating public health information relevant to assigned areas, staying informed about trends in community health needs.
  • Supporting research initiatives and applying findings to enhance evidence-based care, while seeking opportunities to integrate new scientific insights into educational programs.
  • Independently developing and assessing a personal professional development plan.
  • Providing orientation and training for new nursing staff.
  • Referring individuals with abnormal screening results to their primary care providers or appropriate healthcare services.
  • Identifying socioeconomic factors affecting health and well-being, making referrals to relevant agencies, and effectively communicating available services.
  • Administering social determinants of health surveys at community events and making referrals as necessary.
  • Delivering health promotion services in various settings, which may involve physical activities such as lifting and moving equipment.
  • Completing annual departmental competencies and required training modules on time.
  • Performing additional duties and projects as assigned.
  • Adhering to organizational competencies and standards of conduct.

Qualifications

Required Education, Knowledge, Skills, and Abilities:

  • Graduation from a nursing program accredited by the NJ State Board of Nursing.
  • 3 to 5 years of nursing experience.
  • Willingness and ability to travel.
  • Strong written and verbal communication skills.
  • Proficient in computer applications, including Microsoft Office and Google Suite.

Preferred Education, Knowledge, Skills, and Abilities:

  • Bachelor of Science in Nursing (BSN).
  • Bilingual capabilities.
  • Experience in community health.

Licenses and Certifications Required:

  • Current NJ State Professional Registered Nurse's license.
  • Valid NJ driver's license.
  • Annual CPR certification.
