Job Responsibilities:

As a Physical Therapist at East Bay Post Acute, your key responsibilities will include:

  • Developing effective treatment plans and obtaining approval from referring physicians;
  • Treating patients according to physician-approved plans;
  • Collaborating with the nursing department to train Restorative Aides;
  • Supervising Physical Therapy Assistants in direct patient care and related activities, adhering to state practice acts;
  • Communicating patient progress, problems, and plans with supervisors and other healthcare team members;
  • Participating in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences as needed;
  • Providing in-services training to other staff members in the facility;
  • Maintaining accurate records of evaluations, daily treatment notes, weekly progress notes, recertification documentation, and treatment changes, in compliance with PT Board State Practice Act and governmental/third-party payer requirements;
  • Instructing patient families or nursing staff in maintenance programs and caregiver training prior to discharge from therapy services;
  • Contributing to discharge planning;
  • Recommending and facilitating the ordering of necessary durable medical equipment for patients to enhance independence in mobility;
  • Reporting any issues with department equipment to ensure it remains in good working order;
  • Demonstrating excellent communication and interpersonal skills when interacting with patients, families, staff, and professional colleagues.
Required Qualifications:

To be successful in this role, you must hold a valid Physical Therapist license and possess:

  • Strong clinical skills and knowledge in physical therapy principles and practices;
  • Excellent communication and interpersonal skills;
  • Ability to work effectively in a fast-paced, dynamic environment;
  • Capacity to maintain accurate records and adhere to regulatory requirements.
