CHS Recruiting is seeking a highly skilled Primary Care Geriatric Nurse Practitioner to join their team in Cincinnati, Ohio.

Salary and Benefits

The successful candidate will be offered a competitive salary package, including an annual income of $125,000 - $155,000, with opportunities for RVU bonuses up to $30,000 per year. Additionally, we offer a comprehensive benefits package, including malpractice insurance, health, dental, and vision insurance, AD&D/STD/LTD insurance, 4 weeks paid time off, paid holidays, and an annual CME allowance.

Job Description

As a Primary Care Geriatric Nurse Practitioner, you will serve as the primary care provider for a dedicated panel of geriatric patients. You will conduct health assessments, develop personalized care plans, regularly evaluate patient conditions, and adjust treatment plans as necessary. You will also administer medications, treatments, and other therapeutic interventions as prescribed, perform minor procedures and wound care, and monitor vital signs and patient responses to treatment.

Key Responsibilities:
  • Conduct health assessments to identify patient needs
  • Develop personalized care plans
  • Regularly evaluate patient conditions and adjust treatment plans as necessary
  • Administer medications, treatments, and other therapeutic interventions as prescribed
  • Perform minor procedures and wound care as required
  • Monitor vital signs and patient responses to treatment

In this role, you will have the opportunity to work with a diverse range of patients, including those in skilled nursing facilities, assisted living facilities, independent living facilities, and private homes. We use the eClinicalWorks EHR system, and on-call is limited to phone triage only with very low volume.

Requirements and Qualifications

To be considered for this position, you must have an Ohio NP license, DEA certification, and at least one year of advanced practice experience. Previous experience in geriatrics, long-term care, SNFs, house calls, etc. is highly desirable.
