As a Federally Qualified Health Center in Northern California with over 45 years of dedicated service, LifeLong serves a significant number of patients in socio-economically disadvantaged areas. We provide medical, dental, behavioral health, and school-based services across multiple primary care sites. Our innovative approach includes initiatives like a residential respite program for homeless individuals and free community health services.
Benefits:
Compensation: $197,000-$218,000, based on experience, along with a sign-on bonus and comprehensive benefits package, including medical, dental, vision, Flexible Spending Accounts, retirement savings plan, loan repayment programs, and commuter benefits.
Responsibilities:
- Deliver preventive, urgent, and primary healthcare services.
- Conduct clinical assessments, order diagnostic evaluations, and develop care plans.
- Provide education and counseling to patients and their families.
- Coordinate follow-up care and manage patient records for clinic reporting and quality improvement.
- Engage in various settings, including health centers and supportive housing sites.
- Participate in clinical team meetings and contribute to quality improvement initiatives.
Qualifications:
