Job Summary

We are seeking a skilled Physical Therapist to join our team at Pathwell Health. As a Physical Therapist, you will play a vital role in helping patients recover from injuries or illnesses and regain their independence.

Key Responsibilities
  • Develop and implement individualized rehabilitation plans to improve mobility, strength, and function
  • Conduct thorough assessments and evaluations to identify patient needs and goals
  • Collaborate with patients, families, and healthcare teams to ensure effective care and communication
  • Provide education and training on exercise programs and therapeutic procedures
  • Document patient progress and outcomes in a timely and accurate manner
Requirements
  • Graduation from an approved Physical Therapy program
  • Current and valid California Physical Therapist license
  • Recent work experience in home health or a related field
  • Strong clinical assessment, organizational, and communication skills
  • Ability to work well under pressure and prioritize tasks effectively
Preferred Qualifications
  • Bilingual (Spanish) or proficiency in another language
  • Experience working with diverse patient populations
  • Knowledge of current physical therapy practices and techniques
Work Environment

The Physical Therapist will work in a fast-paced home health setting, providing care to patients in their homes. The ideal candidate will be able to work independently and as part of a team, with strong communication and interpersonal skills.
