This MDS Coordinator position requires a detail-oriented and organized professional who can effectively manage multiple tasks and priorities. As a key member of our healthcare team, you will be responsible for coordinating the MDS process, ensuring accurate and timely completion of resident assessments, and collaborating with facility teams to develop individualized care plans.

Responsibilities:

  • Assess resident needs and coordinate the MDS process, including reviewing charts, conducting interviews, and gathering information from facility staff and residents
  • Ensure accurate and timely completion of MDS assessments, including coding and documentation
  • Collaborate with facility teams to develop and implement individualized care plans
  • Monitor and maintain accurate data entry into our computerized program
  • Participate in quality assurance activities and contribute to ongoing process improvements

Requirements:

To succeed in this role, you will need:

  • A current RN licensure in the state of Iowa or compact license; a minimum of two years' experience in long-term care may be considered with exceptional qualifications or equivalent combination of education and experience
  • AANAC (RAC-CT) certification or willingness to obtain within 90 days of hire
  • Clean background checks and certifications as required by law
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
