We are seeking a highly skilled Occupational Therapist to join our team as a PRN member. As an occupational therapist, you will provide comprehensive care to patients in their homes, promoting independence and enhancing quality of life.

The ideal candidate will have excellent communication skills, a strong passion for delivering patient-centered care, and the ability to work effectively in a fast-paced environment.

  • Responsibilities:

    1. Conduct thorough evaluations of patients' daily living and work-related skills, assessing their needs and setting measurable objectives with the rehabilitation team.
    2. Design community reintegration activities to assist patients in physical and cognitive reconditioning, coordinating the plan with the interdisciplinary team.
    3. Recommend and/or design special adaptive equipment for patients to improve function in their home, community, and/or work environments.
    4. Develop retraining programs and train staff and family members to carry out the retraining program, including dressing, feeding, grooming, hygiene, and community re-entry skills.
    5. Provide training to patients and, as appropriate, all others involved in their rehabilitation, analyzing and documenting the results.
    6. Report progress to the rehabilitation staff, patients, and family members, observing/documenting and reporting changes in patients' conditions to the Clinical Coordinator/designee, attending physician, and other team treatment team members as appropriate.
    7. Participate in the team process, providing input and effective treatment strategies, attending case conferences, and family educational meetings.
