Job Title: Patient Care Coordinator

Job Summary:

We are seeking an experienced Patient Care Coordinator to join our team. In this role, you will coordinate patient care activities, communicate with healthcare teams, and promote a positive work environment that supports our mission and values.

Estimated Salary Range: $55,000 - $70,000 per year, based on qualifications and experience.

Key Responsibilities:

  • Coordinate patient care activities, ensuring timely and effective delivery of services.
  • Communicate effectively with healthcare teams, residents, patients, and family members.
  • Monitor and report any changes in patient condition or needs to healthcare teams.
  • Participate in ongoing education and training programs to stay current with industry developments and best practices.
