Hospice Care Specialist

Hospice Plus, a leading provider of hospice and palliative care, is seeking a compassionate Registered Nurse to join our team.
As a Hospice RN, you will play a vital role in delivering high-quality patient care and support to individuals and their families during a challenging time.
Key responsibilities include:
- Providing direct patient care services and supportive care to patients and their families
- Acting as a liaison between physicians, patients, and the hospice team
- Evaluating patient and family needs and assisting in the preparation and execution of the plan of care
- Maintaining accurate and up-to-date patient clinical records
- Participating in quality assessment and performance improvement programs
To be successful in this role, you will possess:
- 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Compassion, empathy, and a commitment to delivering exceptional patient care
