Job Summary

Under the direction of a Registered Nurse and/or designee, and in compliance with state statutes and regulations governing the practice of nursing, the LPN will provide direct, hands-on, high-quality nursing care in accordance with established policies, procedures, and physician orders.

Responsibilities
  • Deliver high-quality patient care, adhering to established standards and protocols.
  • Conduct patient/resident teaching, addressing their unique needs and standards of care.
  • Accurately document assessments, care, teaching, and observations in the medical record.
  • Develop, implement, evaluate, and update individualized interdisciplinary care plans.
  • Supervise and monitor Medication Aides, ensuring appropriate action for any medication effects or interactions.
  • Assist physicians during rounds, exams, and treatments as needed.
  • Train, evaluate, and monitor subordinates, ensuring adherence to policies, procedures, and regulations.
  • Foster a cooperative and respectful environment with team members, families, visitors, and volunteers.
  • Conduct nursing rounds, consult with medical staff, and communicate patient/resident care needs during shift changes.
  • Assist with the admissions/referrals process and pre-admission reviews/assessments.
  • Maintain confidentiality of department, patient/resident, and employee information per privacy laws and regulations.
  • Inform relevant parties of any change in patient/resident condition and initiate appropriate follow-up.
  • Report any suspicion/allegation of abuse, neglect, or misappropriation of property immediately.
  • Participate in the admission, transfer, and dismissal of patients/residents, completing all related documentation.
  • Treat patients/residents, coworkers, and the public with dignity and respect at all times.
  • Mentor new employees and agency personnel, ensuring they function within their scope of practice.
  • Encourage and support patient/resident independence and personal choices.
  • Investigate and address complaints or suggestions from patients/residents, coworkers, or the public.
  • Promote and maintain a clean and safe environment, and address malfunctioning equipment promptly.
  • Respond to medical and psychiatric emergencies and adhere to safety protocols.
  • Attend and participate in staff development and other educational programs.
  • Engage in Quality Assurance Performance Improvement (QAPI) activities.
  • Represent Syra Health positively to community agencies, nursing schools, and the general public.
  • Maintain regular, predictable, and consistent attendance, and perform other duties as assigned.
Requirements
  • Diploma from an accredited college or university in nursing.
  • Current LPN license from Nebraska or a compact state recognized by the Nebraska Board of Nursing.
  • Certification in basic life support/cardiopulmonary resuscitation (CPR) or complete certification within the required timeframe.
  • Preferred experience in healthcare environments including acute care, skilled rehabilitation, skilled nursing, and/or long-term care.
  • Preferred experience using an Electronic Health Care Record system.
  • Completion of pre-employment drug screen, criminal records check, and health review including physical abilities assessment.
  • Annual health review completion.
  • Current CPR
  • Covid Vaccination or Declination/Waiver
