Job Description
We are seeking a highly skilled and experienced Registered Nurse (RN) to join our team as a Home Health Care Nurse. As a key member of our care team, you will be responsible for providing hands-on nursing care to patients in their homes. This includes assessing patient needs, developing and implementing personalized care plans, and collaborating with other healthcare professionals to ensure seamless care delivery.
Required Skills and Qualifications
To be successful in this role, you must have a valid RN license, at least one year of experience in home health care or a related field, and strong communication and interpersonal skills. You should also be able to lift up to 50 pounds, bend, stoop, and stand for long periods. Prior experience with electronic medical records and familiarity with local community resources are a plus.
