Medical Care Specialist

We are seeking a skilled Medical Registered Nurse to join our team at VITAS Healthcare. As a key member of our admissions team, you will play a vital role in ensuring the highest quality care for our patients and their families.
Key Responsibilities- Evaluate patients for hospice appropriateness and analyze data to facilitate admission orders.
- Develop effective relationships with referral sources and internal stakeholders to support high-quality care.
- Utilize interpersonal skills to educate patients and their families about hospice and palliative care.
- Stay up-to-date on the principles of intake, admission, and transition to care processes.
- Proficiently use VITAS' information system and mobile technology platforms.
- Previous experience in hospital, physician practice, LTC, or ALF settings.
- Bachelor's degree preferred.
