About the Role:
At St. Luke's Health System, we are seeking a highly skilled and detail-oriented Pharmacy Technician to join our Transitions of Care Pharmacy team. As a Medication Historian and Clinical Pharmacy Technician, you will play a critical role in ensuring the accuracy and completeness of patient medication information.
Key Responsibilities:
- Perform independent work accurately with attention to detail, gathering information about patients' medications directly from the patient or their family in a professional and courteous manner.
- Identify patients being admitted through the emergency department and other points of entry, and collect and compile a complete list of patient's home medications, including over-the-counter drugs and herbal supplements.
- Input all medication history information into the electronic health record, and keep a record of all work processed for intervention tracking and reporting.
- Actively participate in staff meetings for the purpose of disseminating information and monitoring staff needs, concerns, and suggestions.
- Practice at the top of your license, expand your skill set, and grow your pharmacy functionality.
Requirements:
- High school diploma or equivalent (GED)
- Valid Idaho Pharmacy Technician License
- Preferred Qualifications: Two (2) + years' experience, Certified Pharmacy Technician (CPhT)
