Job Overview

Position Summary:

The Clinical Pharmacist is responsible for reviewing physician medication orders to ensure the appropriateness of medications, dosages, potential interactions, and administration routes. This role involves making therapeutic recommendations to physicians and facility staff as necessary.

  • This position is a contract opportunity with the possibility of transitioning to a permanent role based on attendance, performance, and organizational needs.
  • Full-time employees are eligible for benefits after 90 days, including health, dental, vision, life insurance, and short-term disability coverage.
  • A 401(k) plan with company matching is available for full-time employees after one year of service.

Position Details and Compensation:

  • Location: Vinton, VA – On-site position
  • Compensation: $64 per hour
  • Availability Requirements:
    • Monday to Friday | Some weekends | Shifts from 12:30 PM to 9:00 PM or 2:30 PM to 11:00 PM
    • 7 days on/7 days off | Monday to Friday 10:00 AM to 9:00 PM and Saturday/Sunday 9:00 AM to 7:00 PM

Key Responsibilities:

  • Maintain a comprehensive patient/resident database, including current and historical medical information, relevant clinical and laboratory data, and concurrent medications.
  • Ensure appropriate laboratory tests for monitoring high-risk conditions are ordered, assessed, and discussed with the healthcare team.
  • Collaborate with healthcare professionals to provide clinical and technical support as required.
  • Document all communications with physicians, nurses, and other healthcare team members in the medical record.
  • Implement policies and procedures for the safe procurement, storage, distribution, and disposal of medications and biologicals.
  • Maintain all necessary documentation in compliance with federal and state regulations.
  • Report medication-related errors to the Pharmacy Manager.
  • Assist facility staff with safe and effective methods for managing medications.
  • Perform additional tasks as assigned.

Qualifications:

  • Bachelor's Degree in Pharmacy required; PharmD preferred.
  • Mandatory attendance for the initial 90 days.
  • Minimum of 1 year of pharmacist experience, preferably in Long-Term Care.
  • Must hold a valid pharmacist license in the state of Virginia.
