We are seeking a compassionate and skilled Registered Nurse to join our team at CHP Home Care & Hospice. As a Home Health and Hospice RN, you will provide high-quality, patient-centered care to individuals in their homes, promoting comfort, dignity, and well-being.
Key Responsibilities- Deliver one-on-one care to patients in their homes, including routine nursing procedures, observation, assessment, medication management, and wound care.
- Monitor patients' conditions and assess their needs to provide the best possible care and advice.
- Collaborate with physicians and nurses to devise individualized care plans for patients.
- Perform routine procedures, such as blood pressure measurements and administering injections, and fill in patients' charts.
- Adjust and administer patients' medication and provide treatments according to physicians' orders.
- Inspect facilities and maintain excellent hygiene and safety standards.
- Provide on-call visits in emergencies and foster a supportive and compassionate environment for patients and their families.
- Proven experience as a Registered Nurse.
- Valid Ohio nursing license.
- Driver's license and reliable, insured vehicle.
