We are seeking a skilled Licensed Practical Nurse (LPN) to join our Home Health team at AccentCare, Inc. As a Home Health LPN, you will play a vital role in providing high-quality patient care and support to our patients in their homes.
Key Responsibilities- Provide nursing visits to patients in their homes, following treatment plans prescribed by physicians.
- Maintain accurate and complete medical records, including documentation of patient interactions and treatment outcomes.
- Collaborate with healthcare teams to ensure seamless care coordination and patient satisfaction.
- Participate in ongoing education and training to stay up-to-date on best practices and industry developments.
- One year of clinical experience and one year of home health experience.
- Ability to provide nursing visits according to treatment plans and maintain accurate medical records.
- Graduate of an accredited practical or vocational nursing program.
- Currently licensed Practical Nurse or Licensed Vocational Nurse in the state of agency operation.
- Valid CPR certification.
- Licensed driver with ability to travel to all business locations.
- Competitive salary range: $28 - $36 per hour.
