RN Community Care Specialist

This role is designed to provide care and consultation for patients with specific needs. The RN Community Care Specialist will work under the guidance of certified wound care nurses and physicians to assess patient needs, develop care plans, and educate patients and healthcare teams on evidence-based practices.
Responsibilities- Assess physical and educational needs of patients undergoing ostomy surgery or continent diversions, and patients with wounds, fistulas, incontinence, percutaneous tubes or pressure ulcers.
- Communicate patient needs to healthcare teams and implement and evaluate care management plans.
- Coordinate care in an efficient and cost-effective manner.
- Assist with education of patients, families, and staff on skin and wound care practices.
To be successful in this role, you will need:
- An Associate degree in Nursing; Bachelor's degree preferred.
- Experience in skin and wound care.
The estimated annual salary for this position is $80,000-$110,000 based on location and experience. Additionally, Prisma Health offers a range of benefits, including health insurance, retirement plans, and paid time off.
