As a Registered Nurse Care Manager at Places for People, you will play a vital role in providing integrated health and nursing services to individuals in the St. Louis community. This is a community-based position, requiring approximately 50% travel within the St. Louis area.
Key Responsibilities
Collaborate with treatment team members to provide comprehensive care to clients with co-occurring mental health and chronic health conditions.
Conduct annual physical health assessments with clients, including physical health recommendations.
Educate clients and team members on medications, side effects, nutrition, and management of chronic physical health diagnoses.
Provide basic first aid and assessment/intervention in medical emergency situations.
Collaborate with clients and team to create measurable physical health goals for treatment plans.
Communicate health indicator data received from healthcare providers to team and clients, and revise treatment plans accordingly.
Coordinate enrollment of team clients in healthcare services.
Collaborate with Physical Health Integration Team (PHIT) for support and to enroll clients in primary behavioral health care projects.
Participate in team meetings, treatment planning meetings, and medical services meetings.
Advocate with external healthcare providers for clients to receive standard of care and communicate health goals to primary care providers.
Complete annual training requirements as outlined by agency requirements and supervisor.
Requirements
Registered Nurse with six months of psychiatric nursing experience.
CPR certification.
Personal vehicle, Chauffeur's License, and valid auto insurance.
