Clinical Nurse Case Manager

As a Clinical Nurse Case Manager, you will play a vital role in conducting comprehensive assessments to enhance patient care management. Your responsibilities will include managing a designated caseload and performing in-depth evaluations of patients' functional, medical, behavioral, and psychosocial needs.
Key Responsibilities
1. Conduct thorough assessments to provide the Interdisciplinary Care Team (ICT) with essential information to meet patient needs.
2. Develop individualized care plans based on patients' clinical, behavioral, and social requirements, addressing any barriers to effective care.
3. Assess patients' health status competently, ensuring they receive all necessary medical and supportive services.
4. Adjust care plans as needed to reflect changes in patients' conditions and progress.
5. Facilitate smooth transitions of care through timely communication essential for patient management and discharge planning.
6. Oversee the coordination of services, addressing issues related to admissions, discharges, team conferences, patient education, payer relations, and overall fiscal management.
7. Ensure the delivery of cost-effective treatment while adhering to patients' rights regulations.
Qualifications
To be considered for this role, candidates must possess:
- A valid UAS Certified RN license in the state of New York.
- Consideration for new graduates and candidates with home care management experience without UAS certification.
- A Bachelor's Degree in Health Education or a related field, or a combination of relevant experience and education.
