Clinical Pharmacist

Mission Box Solutions is seeking a dedicated and experienced Clinical Pharmacist to join our team. As a key member of our healthcare team, you will play a critical role in ensuring safe, cost-effective, and appropriate medication use.
Key Responsibilities:- Medication Preparation and Dispensing: Prepare and dispense medications per physician orders following established policies, procedures, and protocols.
- Medication Therapy Management: Accurately interpret and transcribe drug orders into the patient medication record, monitor drug therapy for safety, contraindications, and interactions, and provide clinical consultation to healthcare providers.
- Pharmaceutical Care: Offer drug education to patients and their families, supporting optimal medication use and adherence, and participate in clinical interventions, including IV to PO conversions, renal dosing, and pharmacokinetic support.
- Leadership and Collaboration: Supervise and direct pharmacy technicians, ensuring the accuracy of their work, and engage in interdisciplinary rounds and collaborate with healthcare teams to improve patient outcomes.
- Professional Development: Maintain current pharmacist licensure and complete all required continuing education, and ensure compliance with all competency and skill assessments as required by the department.
- Education: Doctor of Pharmacy (PharmD) or BS in Pharmacy.
- Experience: Hospital or acute care experience required.
- Licensure: Pharmacy registration in the state of practice with state licensure within 60 days of start date.
- Certifications: BLS or ACLS and PALS (pediatric practice) certifications within 180 days of start date.
