About the Role

In this critical position, you will play a vital role in supporting individuals in need, providing direct care and stabilization services in a fast-paced, dynamic environment. As a Residential Care Coordinator, you will be responsible for assessing service needs, developing individualized service plans, and providing support to program participants.

Essential Duties and Responsibilities
  1. Develop and Conduct Group Sessions: Facilitate group skills restoration sessions to promote stability and skill development among participants.
  2. Provide Crisis Intervention and Stabilization: Offer intensive case coordination services, including referral and application assistance, to help individuals stabilize and access necessary resources.
  3. Assess and Plan Individual Services: Evaluate service needs and develop tailored service plans to meet the unique requirements of each participant.
  4. Support Program Participants: Process participant intakes, perform initial assessments, prepare admission documentation, provide orientation to the facility, and screen for contraband.
  5. Medication Administration: Assist participants with self-administering prescribed medications.
  6. Data Management: Input assessment and other data into the electronic records system.
  7. Enforce Program Policies: Uphold program policies and health and safety regulations with participants.
  8. Coordinate Discharges: Facilitate discharges and arrange follow-up care and services with other agency programs.
Competencies and Requirements
  • Knowledge of Behavioral Health Principles: Possess considerable knowledge of group and individual skills restoration principles, crisis intervention methods, and medical providers.
  • Effective Communication: Demonstrate ability to establish rapport with participants, conduct effective interviews, observe, assess, and record behaviors and functioning.
  • Team Collaboration: Show capacity to maintain effective working relationships with staff from private and public agencies and organizations.
  • Adaptability: Ability to function calmly in stressful situations and adapt to changing circumstances.
