Job Description

We are seeking a skilled Occupational Therapist/Program Manager to oversee our HUD Older Adult Home Modification grant, a three-year program.

As an Occupational Therapist/Program Manager, you will have the opportunity to make a difference in the lives of others by helping them live independently and improving their quality of life.

You will engage with patients and teach them to regain their ADL and recommend assistive devices that may result in modifying their homes to make it safe and accessible.

You will oversee day-to-day operations of the home modification program, conduct on-site reviews of projects, assist with enrollment and recruitment, verify income and coordinate with contractors and stakeholders.

You will perform administrative duties to document patients' activities and progress, revise care plans, record data and do billing.

Responsibilities
  • Conduct home visits and create care plans with the multidisciplinary team
  • Engage with patients and teach them to regain their ADL
  • Recommend assistive devices to modify homes for safety and accessibility
  • Oversee day-to-day operations of the home modification program
  • Conduct on-site reviews of projects
  • Assist with enrollment and recruitment
  • Verify income and coordinate with contractors and stakeholders
  • Perform administrative duties to document patients' activities and progress
  • Revised care plans, record data and do billing
Requirements
  • Bachelor or Master's degree in an occupational therapy curriculum accredited by ACOTE standards
  • Valid certification from the National Board for Certification of Occupational Therapy (NBCOT)
  • Occupational Therapist license for State of Hawaii in good standing
  • Minimum of five (5) years completing Environmental Accessibility Assessments or possess specialized certifications

Your proposal/sealed bid must include your qualifications, experiences and fees for the following services.

  • 1-2 assessments/week (travel time, assessment, write-up hours)
  • Reviewing contractor bids and obtaining landlord approvals (2 hours)
  • Meetings to coordinate home modifications with project team, contractors, families, landlords (1-2 hours/consumer)
  • Follow up visits (2 hours/consumer)
  • Close out visits, including post-home modification caregiver training (4 hours)
  • Informal team meetings as needed (1-2 hours/month)
  • Administrative tasks (invoice, review documents) (1 hour/month)

Additional activities include:

  • Educational outreach with Palolo Chinese Home (1 hour/month)
  • Bi-monthly team meeting (2 hours/month)
  • Intake and triage assistance (2 hours/month)
  • HUD requested training in Washington D.C. six days per year
