Nurse Navigator

Cedars-Sinai is seeking a skilled Nurse Navigator to join our Congenital Heart Program. As a key member of our team, you will play a vital role in providing exceptional patient care and support to families affected by congenital heart disease.

• Establish and revise patient care plans in collaboration with the multidisciplinary team.
• Coordinate patient care activities and manage clinical data throughout the care continuum.
• Provide emotional support, coping support, and care for patients and their families.
• Coordinate support services within the health system and community to meet patient needs.
• Communicate effectively with patients, families, and healthcare teams.
• Lead quality improvement activities to enhance patient care and outcomes.

• Bachelor's degree in nursing required; Master's degree preferred.
• Current, unrestricted California RN license required.
• 3 years of experience in a pediatric setting, preferably in congenital heart or cardiology.
• Strong customer satisfaction, analytical, and problem-solving skills.
• Able to use computer software and operate basic office equipment.
