About the Role

CarePartners is seeking a skilled Registered Nurse Home Care PACE to join our team. As a key member of our interdisciplinary team, you will provide coordinated skilled nursing care to patients in their homes, ensuring high-quality patient outcomes and a positive patient experience.

Responsibilities
  • Assess patients' physical, psychosocial, and environmental needs, identifying areas for improvement and developing effective care plans.
  • Collaborate with the interdisciplinary team to establish and achieve patient goals, ensuring seamless care coordination and communication.
  • Supervise and provide clinical direction to home health aides and LPNs/LVNs, promoting quality and continuity of services.
  • Participate in on-call rotation and emergency call, providing timely and effective support to patients and families.
Requirements
  • Basic Cardiac Life Support certification within 30 days of employment start date.
  • Registered Nurse licensure.
  • Valid driver's license.
  • Associate Degree in Nursing.
