The Rehabilitative Care Specialist engaged with Elite Home Health & Hospice is tasked with delivering comprehensive physical therapy services while ensuring compliance with all stipulations outlined in the employment agreement.
DUTIES & RESPONSIBILITIES
- Responsible for the continuous interdisciplinary evaluation and formulation of a personalized care plan in collaboration with the patient, their representative (if applicable), and caregivers;
- Offers counseling to patients, caregivers, and families;
- Facilitates education for patients and caregivers;
- Documents clinical notes and progress reports based on goal achievement;
- Maintains communication with the physician overseeing the home health care plan and other healthcare professionals as necessary;
- Delivers physical therapy services to patients in accordance with a physician's written care plan, which may include:
- Assessing and evaluating therapeutic, rehabilitative, and functional status, as well as contributing to the overall care plan. Evaluates the home environment.
- Evaluating muscle strength, mobility, gait, and range of motion with potential for rehabilitation.
- Directing physical therapy interventions.
- Instructing patients and their families/caregivers on the use and maintenance of therapeutic devices.
- Identifying priority needs for physical therapy services.
- Reporting to the physician regarding the patient's response to treatment or any changes in condition.
- Initiating physical therapy programs and guiding other personnel and/or family members in specific phases of therapy, as well as clarifying the therapy goals during case discussions.
- May provide training for patients in the use of prosthetic devices.
- Recognizes patient and family/caregiver needs for additional home health services and makes referrals as necessary.
- Contributes to discharge planning for patients.
- Conducts in-service educational sessions for nursing staff as required.
- Engages in peer consultation processes.
- When therapy is the sole skilled service, supervises and evaluates home health aide care biweekly.
- Oversees Physical Therapy Assistants in accordance with organizational policies and state regulations.
- Participates in the quality assessment and performance improvement initiatives of the home health agency and in-service training programs.
Job Type: Part-Time
Pay: $55.00 per hour

JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)
- Must be licensed to practice as a physical therapist in the State of Idaho and the State of Washington.
- Must have passed a state-approved examination for physical therapists and possess a degree from a baccalaureate or master's program in physical therapy accredited by the Commission on Accreditation in Physical Therapy Education (CAPTE) or its successor.
- At least two years of relevant experience as a physical therapist is required, with a preference for community/home health experience.
- Exhibits strong verbal and written communication and organizational skills.
- Must hold and maintain current CPR certification.
- Must possess a valid driver's license and have access to a vehicle that is insured per state or organizational requirements and is in good working condition.
- COVID-19 vaccination is mandatory, with accommodations considered for disability and religious reasons.
