About the Role
As a Primary Care Physician at Oak Street Health, you'll be part of a collaborative team working together to deliver exceptional patient care. Your role will involve assessing and diagnosing patients, overseeing primary care services, and prescribing treatments as needed.
You'll have the opportunity to work closely with our care teams to develop and implement effective treatment plans, ensuring that our patients receive the highest quality of care possible.
Responsibilities
- Assess and diagnose patients to develop personalized treatment plans
- Oversee primary care services, including medical consultations and medication management
- Collaborate with care teams to ensure seamless delivery of patient care
- Maintain accurate and up-to-date patient records, using Canopy and Greenway electronic health record systems
- Participate in ongoing education and training to stay current with best practices and emerging trends in primary care
Qualifications and Requirements
- Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) graduate with active, non-probationary state licensure
- Board certification in Internal Medicine or Family Medicine (or board-eligible)
- Active DEA license and USA work authorization
- Excellent communication and interpersonal skills, with a passion for delivering outstanding patient care
Preferred Qualifications
- Fellowship training in Geriatrics
- Experience practicing medicine among geriatric populations
- Experience operating in value-based healthcare models
- Experience working in collaborative settings to drive positive health outcomes
