Salary: $55,000 - $65,000 per year (dependent on experience)
Job SummaryThis Behavioral Health Associate II role is an exciting opportunity for a passionate and skilled professional to join our team. As a key member of our community program, you will be responsible for providing comprehensive support to members, enabling them to achieve their goals and improve their quality of life.
The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work collaboratively with members, staff, and external agencies. You will be committed to delivering high-quality services, prioritizing the safety, health, and well-being of our members.
Key Responsibilities:
- Assist members in identifying and progressing toward achievement of member chosen goals.
- Empower members in meeting their goals, providing vocational support as needed.
- Collaborate with staff and members in developing and implementing activities, programming, and groups.
- Foster effective communication and cooperation among agencies.
- Participate in client medication outreach, coordinating communication between doctor, client, and pharmacy.
- Educate community and other agencies about our services and programs.
- Transport members to and from appointments and activities, as well as conduct field visits/medication outreach.
- Provide prompt intervention in crisis situations, ensuring member safety and notifying relevant parties when necessary.
- Offer on-the-spot support to members, aligning with the program's philosophy.
- Maintain accurate records and documentation, including progress notes, activity reports, and logs.
- Support clients in accessing and maintaining suitable housing.
- Prioritize the safety, health, and well-being of members.
- Adhere to program standards and requirements, working closely with the Program Director or designee.
