Job SummaryWe are seeking a skilled Licensed Practical Nurse (LPN) to join our Transitional Care Unit team. As an LPN, you will play a vital role in providing comprehensive care to patients, working collaboratively with the interprofessional team to achieve patient-centered outcomes.
Responsibilities- Provide direct patient care, including administering medications, performing wound care, and monitoring vital signs.
- Collaborate with the healthcare team to develop and implement individualized care plans.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
- Supervise and mentor certified nursing assistants and other auxiliary staff.
- Active NJ LPN license and BLS certification.
- Minimum 2 years' experience in post-acute care settings (skilled nursing facility, rehabilitation, long-term care).
- Excellent communication and interpersonal skills.
