Key Responsibilities
  • Assess and Plan Patient Care: Utilize the nursing process to assess, plan, implement, and evaluate patient care, ensuring that each patient receives individualized attention and treatment.
  • Collect and Analyze Data: Collect, analyze, and interpret data and information from healthcare members to identify potential nursing diagnoses and develop effective care plans.
  • Implement and Evaluate Care Plans: Implement nursing interventions according to identified priorities, plan of care, and hospital policies, and evaluate the effectiveness of care plans to ensure optimal patient outcomes.
  • Collaborate with Healthcare Team: Work collaboratively with our healthcare team, including physicians, nurses, and other healthcare professionals, to ensure seamless communication and coordination of patient care.
  • Document Patient Information: Accurately and thoroughly document patient information, including medical history, treatment plans, and progress notes, to ensure continuity of care.
Requirements
  • Current LPN License: Possess a current Licensed Practical Nurse (LPN) license for the state in which you practice.
  • Education and Experience: One year of prior LPN experience preferred, with a strong foundation in nursing principles and practices.
  • Skills and Qualifications: Possess excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced healthcare environment.
