Senior Physical Therapist, Adult Outpatient Specialist

As a Senior Physical Therapist in our Adult Outpatient department, you will provide high-quality physical therapy services to patients, working independently and as part of a team to achieve optimal patient outcomes. Your expertise will be essential in developing and implementing patient care programs, collaborating with other healthcare professionals, and providing education to patients and families.
Key Responsibilities:- Evaluate patients and interpret evaluation findings to determine the nature and extent of dysfunction.
- Establish treatment goals and plans treatment to achieve established goals.
- Initiate discharge planning for physical therapy and implement the treatment plan through direct treatment of the patient.
- Delegate a portion of the treatment plan to appropriate supportive personnel and coordinate the treatment program with other healthcare personnel as needed.
- Provide advice and education to patients and families, and regularly reassess clinical signs and symptoms to determine the effectiveness of treatment and progress towards goals.
- Discharge patients from physical therapy when they have received maximum benefit from physical therapy, and provide accurate and timely documentation of patient management.
- Minimum two years of experience in physical therapy.
- Graduate of an accredited physical therapy curriculum with a bachelor's, master's, or doctoral degree with certification in physical therapy or foreign-trained equivalent.
- Physical Therapist License (California) required at hire.
- Basic Life Support required at hire.
- National Provider Identifier required at hire.
- Certified Clinical Instructor for PT students.
