**Job Title:** Geriatric Care Specialist

**About the Role:

We are seeking a highly skilled and compassionate Geriatric Care Specialist to join our team at Northampton County, PA. As a key member of our Aging Services department, you will play a vital role in developing care plans for older adult clients through our county Area Agency on Aging program.

About the Job:

  • Perform Functional Eligibility Determinations as per State Program requirements for home and community-based services and nursing facility placement.
  • Complete assessments via face-to-face contact with consumers.
  • Obtain medical, psychological, and/or psychiatric data necessary to determine consumer membership in pre-admission assessment programs.
  • Communicate determinations to OPTIONS consumers and complete assessments in the State-mandated electronic information system using remote access technology.
  • Maintain liaison with area hospitals, nursing homes, and agencies to expedite information and paper flow.
  • Participate in unit meetings, supervisory conferences, and training/in-service programs as scheduled.
  • Counsel consumers about alternatives in the community, work with discharge planners in skilled nursing facilities, and function as a consumer's advocate.

Requirements:

  • 6 months of experience as an Aging Care Manager 1 or a County Caseworker 1, or successful completion of the County Social Casework Intern program, or 6 months of professional human services or nursing experience and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field.
  • 12 semester credit hours of college-level courses in sociology, social welfare, psychology, gerontology, or other related social sciences.
  • A valid and current motor vehicle operator's license with an acceptable driving record.

Working Conditions:

  • The noise level in the office is moderately quiet, and employees may also work in other environments such as schools, private homes, etc.
  • Employees must be able to lift and/or move up to 25 pounds.

Salary Range: $

$66,317 - $94,695 annually.
