Behavioral Health Case Manager II Position

Job Summary:
The Behavioral Health Case Manager II is a critical role within Elevance Health, responsible for providing comprehensive case management services to members with complex behavioral health and substance abuse needs. This position requires a strong understanding of clinical assessment, care planning, and coordination of services to ensure optimal outcomes for our members.
Key Responsibilities:
• Conduct thorough assessments to identify individual needs and develop personalized care plans
• Coordinate with internal and external resources to meet the needs of members, including healthcare providers, community agencies, and family members
• Monitor and evaluate the effectiveness of care plans, making adjustments as needed to ensure optimal outcomes
• Collaborate with other healthcare professionals to ensure seamless care coordination and communication
• Participate in cross-functional teams to drive business outcomes and improve member satisfaction
Requirements:
• Master's degree in social work, counseling, or a related field, or a degree in nursing with a minimum of 3 years of clinical experience
• Current active unrestricted license to practice as a health professional within the scope of licensure in applicable states or territory of the United States
• Previous experience in case management and telephonic and/or in-person coaching with members with complex psychiatric and substance abuse disorders
Preferred Skills:
• Experience in health coaching and motivational interviewing techniques
Work Environment:
This is a remote position, requiring the ability to work independently and collaboratively as part of a team. The ideal candidate will reside in the Beckley or Princeton area and be willing to interface with members and providers in person 2-3 days per week.
