We are seeking a qualified professional to fill a vital role in a respected healthcare facility:

  • Must be Board-Certified in Family or Internal Medicine
  • Fellowship training in Geriatric Medicine is required
  • Standard work hours from Monday to Friday, 8 am to 5 pm
  • Responsible for delivering comprehensive Primary Care services to patients aged 60 and older
  • Collaborate with a multidisciplinary team that includes 2 Geriatric Physicians, 2 Advanced Practice Providers (APP's), a Pharmacist, a Social Worker, and a Community Case Manager
