Job Title: Registered Nurse - Med Surg Tele Per Diem

Job Summary:

Baystate Health, a nationally recognized leader in healthcare quality and safety, is seeking a Per Diem Registered Nurse (RN) to join our Med/Surg/Tele team in Greenfield, MA. Responsibilities:

  • Provide age-appropriate care to patients, demonstrating knowledge of developmental stages and applying developmental theories and concepts when planning and implementing care.
  • Perform a comprehensive nursing assessment, using professional judgment to obtain selective data and arrive at nursing diagnoses and discharge planning needs.
  • Implement a detailed, realistic plan of care specific to each patient's identified needs, developing and updating a plan of care with measurable and achievable goals.
  • Analyze the psycho-social impact of illness and hospitalization on individuals and families, collaborating and negotiating with patients and families to arrive at an agreed-upon plan of care.
  • Implement the individualized plan of care, demonstrating proficiency in clinical nursing skills and prioritizing plan of care accurately.
  • Provide formal and informal teaching relevant to health care needs of patients, ensuring patients/families are informed concerning treatments, procedures, and hospital policies and routines.
  • Effectively implement and revise the discharge plan, initiating interdisciplinary discharge planning at time of admission and preparing patient/family for changes in the health care delivery system.
  • Establish a pattern of ongoing evaluation and modification of patient care interventions, determining the effectiveness of nursing care and evaluating whether selected nursing interventions were appropriate to reach set goals.
  • Accurately and concisely communicate pertinent patient information, maintaining confidentiality and documenting patient care and patient responses promptly, clearly, and thoroughly on appropriate forms.
  • Act as an advocate for patient/families, intervening consistently in situations where patient integrity is threatened and supporting and encouraging patient/family decisions maintaining respect for life and dignity.
  • Assume accountability for management of patient care, providing direct supervision to other health team members and promoting effective management of patient care.
  • Identify own professional educational needs, assessing each patient/family's comprehensive learning needs in relation to health and illness and individualizing standardized teaching plans to meet specific needs of patient/family.

Requirements:

  • MA Registered Nurse License
  • American Heart Association BLS CPR

Preferred Qualifications:

  • Minimum of 1 year of nursing experience
