Job Description

Pinnacle Home Care Inc. is a leading provider of home health care services, dedicated to delivering high-quality patient care in the comfort of their own homes. We are seeking a skilled and compassionate Registered Nurse (RN) to join our team as a Home Health Care Professional.

Key Responsibilities:
  • Conduct thorough assessments of patients to determine their home care needs, including physical assessments and medical histories.
  • Develop and implement individualized care plans to meet the unique needs of each patient, incorporating therapeutic, preventative, and rehabilitative nursing actions.
  • Administer medications and treatments as prescribed by physicians, and provide direct patient care as defined by state nursing practice acts.
  • Counsel patients and their families on meeting nursing and related needs, and provide health care instructions as appropriate.
  • Identify discharge planning needs and implement plans prior to patient discharge.
  • Act as a Case Manager when assigned, coordinating patient care for assigned caseloads.
Requirements:
  • Associate's or Bachelor's degree in Nursing, with 1-2 years of recent acute care experience in an institutional setting.
  • Current licensure as a Registered Nurse in the state, and current BLS certification.
  • Licensed driver with an automobile in good working order and insured in accordance with the organization's requirements.
  • Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
  • Prolonged or considerable walking and standing, with ability to lift, position, or transfer patients, and lift supplies and equipment.
