About the Role

Rosecrance is seeking a skilled Behavioral Health Access Counselor to join our team. As a key member of our access team, you will play a critical role in providing comprehensive diagnostic assessments and determining medically necessary service needs for clients.

Key Responsibilities
  • Conduct centralized diagnostic assessments, including computerized social histories, mental status, presenting problem, diagnosis, and treatment needs.
  • Determine appropriate levels of care and treatment using center utilization management criteria, develop corresponding ISPs with clients, and coordinate needs with appropriate staff.
  • Identify the need for immediate crisis intervention services and maintain documentation per center quality assurance procedures and clinical performance standards.
  • Assess client needs and strengths, establish a comprehensive plan that addresses support and treatment needs necessary for less restrictive care and stabilization to occur safely.
  • Provide linkage, referrals, and advocacy with community resources and community organizations.
  • Cooperate with the work team/center/on accomplishment of strategic objectives and activities.
  • Ensure that clinical treatment needs are met for consumers, enabling them to work towards recovery.
  • Educate community agencies regarding services offered by Rosecrance.
  • Participate in all team meetings and activities as a member of the Access team.
  • Exercise confidentiality in keeping with the Code of Conduct and with the framework of the law.
Requirements
  • Bachelor's in counseling, psychology, social work, or another related field. Master's degree in licensable field strongly preferred.
  • Must qualify as a MHP or higher per Illinois Department of Mental Health Rule 132.
  • Illinois Certified Addiction Counselor (CADC), or licensed in Illinois as a LSW, LPC, or higher, within 2 years of employment.
  • Knowledge of DSM diagnoses and appropriate criteria for each diagnosis.
  • Knowledge of accepted assessment techniques.
  • Ability to quickly establish rapport.
  • Computer proficiency required.
  • Ability to navigate and complete documentation in the electronic healthcare record.
  • Ability to accurately complete required documentation within the prescribed time frames.
  • Self-starter with ability to work independently.
  • Ability to work as a team member.
  • Valid driver's license and ability to meet Agency insurance requirements (designated positions).
  • Must have own transportation (designated positions).
