Job Summary:
Fresenius Medical Care North America is seeking a highly skilled and compassionate Home Dialysis Registered Nurse to join our team. As a Home Dialysis Registered Nurse, you will play a vital role in providing high-quality patient care and education to patients with chronic kidney disease.
Responsibilities:
- Provide comprehensive patient care, including assessment, planning, intervention, and evaluation, to patients with chronic kidney disease.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans.
- Train patients and their families on home dialysis equipment and treatment supplies.
- Monitor patient progress and adjust treatment plans as needed.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Participate in quality improvement initiatives to enhance patient outcomes and facility operations.
Requirements:
- Graduate of an accredited School of Nursing.
- Current appropriate state licensure.
- Minimum of 9 months experience as a Registered Nurse (preferred).
- Chronic Home dialysis or hemodialysis experience (preferred).
- Successful completion of FKC Education and Training requirements for new employees.
- Successful completion and maintenance of CPR BLS Certification.
Working Conditions:
The Home Dialysis Registered Nurse will work in a fast-paced, dynamic environment, providing care to patients in their homes. The position requires frequent travel to patient homes and may involve working in a variety of settings, including patient homes, clinics, and hospitals.
