Mass General Brigham is seeking a highly skilled Senior Mental Health Case Manager to join our dynamic team.

About the Role:

This exciting opportunity allows you to utilize your expertise in case management to deliver exceptional patient care. As a Senior Mental Health Case Manager, you will be responsible for coordinating and networking care with families and outpatient providers, conducting treatment and discharge planning meetings, crisis and safety planning, arranging aftercare for patients who are in treatment for major mental illness and co-occurring substance use disorders.

Key Responsibilities:
  • Develop and implement comprehensive treatment plans in collaboration with interdisciplinary teams.
  • Conduct psychosocial assessments to identify patient needs and develop individualized care plans.
  • Cordainate and network care with families and outpatient providers to ensure seamless transitions and optimal outcomes.
  • Conduct treatment and discharge planning meetings to educate patients and their families on treatment options and discharge instructions.
Requirements:

To be considered for this role, you must possess a Master's Degree and have experience in case management, preferably in an inpatient adult psychiatry setting. Strong communication and interpersonal skills are essential, as well as the ability to work effectively in a fast-paced environment.

Estimated Salary:

$83,000 - $110,000 per year, depending on experience and qualifications.
