Responsibilities
  • Conduct thorough assessments and develop personalized treatment plans for patients in their homes.
  • Provide one-on-one therapy sessions to patients, focusing on improving mobility, strength, and function.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care and achieve optimal outcomes.
  • Stay up-to-date with the latest evidence-based practices and guidelines in physical therapy.
  • Communicate effectively with patients, families, and healthcare teams to ensure clear understanding and expectations.
Qualifications
  • Graduate of a CAPTE-accredited Physical Therapist education program.
  • Current licensure as a Physical Therapist in the state of practice.
  • At least one year of experience in caring for similar patient populations.
  • Basic knowledge of current medications, including modes of administration, actions, and side effects.
  • Ability to navigate and perform basic use of Microsoft Office products.
  • Maintains a reliable mode of transportation and has the ability to drive safely during all day and night hours and in all types of inclement weather.
