Job Summary:

We are seeking a dedicated and compassionate Licensed Vocational Nurse to join our team at St. Gabriel's Hospice and Palliative Care. As a LVN, you will play a vital role in providing high-quality patient care and support to individuals and their families in the San Angelo area.

Key Responsibilities:

  • Provide nursing care to patients of all ages in their place of residence, under the supervision of a registered nurse.
  • Coordinate care with the Interdisciplinary Team, patient, family, and referring agency.
  • Assist in identifying patient needs and developing care plans in conjunction with the RN.
  • Apply concepts of infection control and standard precautions to prevent contamination and transmission of disease.
  • Ensure continuity of care through delivery of quality patient care.
  • Communicate effectively with patients, families, team members, and other healthcare professionals.
  • Monitor assigned cases to ensure compliance with requirements of third-party payors.

Requirements:

  • Graduate of an accredited School of Vocational/Practical Nursing.
  • State license as a Licensed Vocational/Practical nurse, or a multi-state license issued by a member of the National Nurse Licensure Compact (NLC).
  • Current driver's license and valid auto liability insurance.
  • Two years of experience as a Licensed Vocational/Practical Nurse in a clinical care setting, with hospice experience preferred.
