Visiting Nurse Service RN

The Pioneers Medical Center is seeking a skilled and compassionate Visiting Nurse Service RN to join our team. As a key member of our healthcare team, you will provide high-quality patient care in the comfort of patients' homes, focusing on delivering exceptional patient outcomes and experiences.
Key Responsibilities
Perform admission assessments within 48 hours of referral or patient return to residence
Develop and implement individualized care plans for each patient
Assess patient condition, initiate plan of care, and re-evaluate as necessary
Utilize evidence-based pain management techniques and educate patients and families on pain management
Requirements
Diploma, Associate, or Baccalaureate Degree from an accredited school of nursing required
Current, unencumbered Colorado Nursing license required
Current BLS certification required within six months of hire or transfer
Working Conditions
The working environment is a normal clinical setting and patient home setting with exposure to sharps and biological and chemical agents. It is expected that facility safety practices and protocols are followed at all times.
