Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our Med/Surg team on a per diem basis. As an LPN, you will provide high-quality patient care, work collaboratively with our interdisciplinary team, and contribute to a positive and supportive work environment.

Responsibilities
  • Assess, plan, implement, and evaluate nursing interventions and patient care plans in collaboration with the Registered Nurse.
  • Provide patient care based on established standards of care, protocols, and policies within the scope of your license.
  • Respect each patient's individuality and provide care that fosters their dignity and positive self-regard.
  • Provide patient and family education based on identified learning needs, readiness to learn, and barriers to learning.
  • Collaborate with the RN, provider, and interdisciplinary team to plan and prepare patients and families for discharge.
  • Delegate and supervise interventions performed by Certified Nurses Aides as needed.
  • Prioritize patient care, seeking assistance as needed from appropriate resources, and recognizing the need for emergency intervention.
  • Document all pertinent data, therapeutic interventions, and patient responses accurately and timely according to established standards.
  • Provide thorough and timely reports on assigned patients to other healthcare team members to ensure integration of services and continuity of patient care.
  • Participate in multi-disciplinary treatment teams to plan, coordinate, implement, and evaluate patient care.
  • Meet with families and significant others to plan appropriate disposition and discharge.
Requirements
  • Current LPN license to practice in MA
  • Current American Heart Association CPR certification card
  • One year of med/surg experience with skilled nursing care and post-op orthopedics experience preferred
  • Previous experience in inpatient hospital or skilled nursing setting preferred
