At Arkansas Hospice, we are seeking a skilled and compassionate RN Case Manager to join our team. As a RN Case Manager, you will play a vital role in providing high-quality patient care and support to our patients and their families.

Responsibilities:
  • Manage patient care functions after initial admission and assignment to a designated RN Case Manager.
  • Assist in the admission process as requested.
  • Interpret and communicate Arkansas Hospice policies and procedures to nursing staff and others.
  • Coordinate and collaborate with interdisciplinary team members to manage patient care plans.
  • Update patient care plans on an ongoing basis and communicate with team members regarding patient progress.
  • Facilitate effective communication with patients, families, and healthcare providers.
  • Monitor pain and symptom control to ensure optimal patient comfort.
  • Coordinate CNA scheduling needs with Patient Care Coordinator/Area Manager or designee.
  • Initiate referrals for hospice-related care and services.
  • Serve as a preceptor for nurses in orientation, students, and trainees.
  • Review documentation of staff as directed and ensure accuracy and timeliness.
  • Participate in interdisciplinary team meetings, case conferences, and other committee meetings as assigned.
  • Actively participate in Performance Improvement activities.
  • Participate in community outreach activities to educate the public about Arkansas Hospice services.
  • Coordinate and provide education on hospice and hospice services for patients, families, and team members.
  • Rotate on-call duty to provide 24-hour service to patients and families as assigned.
  • Maintain strict confidentiality at all times.
Requirements:
  • Graduation from an accredited school of nursing; Bachelor of Science in Nursing (BSN) preferred.
  • Minimum of one year nursing experience in a hospital, home health agency, hospice, or related program.
  • Licensed as a Registered Nurse (RN) in Arkansas.
  • Valid Arkansas driver's license, personal automobile, automobile insurance, and personal cell phone required.
Competencies, Knowledge, Skills, and Abilities:
  • Knowledge of palliative care with emphasis on symptom control associated with terminal illness.
  • Knowledge of the holistic approach toward patient care and the nursing process as it applies to hospice patients and families.
  • Knowledge and acceptance of hospice philosophy and principles of care.
  • Skill in organizing and prioritizing workloads to meet deadlines.
  • Ability to recognize both positive and negative aspects of death and grief.
  • Ability to communicate effectively both orally and in writing.
  • Ability to communicate effectively with co-workers and other customers.
  • Ability to follow basic safety policies and procedures.
  • Ability to use good judgment and to maintain confidentiality of information.
  • Ability to work as a team player.
  • Ability to demonstrate tact, resourcefulness, patience, and dedication.
  • Ability to accept direction and adhere to policies and procedures.
  • Ability to work in a fast-paced environment.
