Responsibilities of the Family Support Coordinator include, but are not limited to:

  • Coordinating and delivering care that is safe, timely, effective, efficient, equitable, and centered on the client.
  • Managing case assignments, developing service plans, reviewing case progress, and determining the Plan of Care (POC).
  • Assisting clients in achieving wellness and independence.

We are looking for candidates who can:

  • Coordinate and deliver client-centered care.
  • Manage case assignments and service plans effectively.
  • Support clients in achieving autonomy and wellness.
  • Facilitate various aspects of care, including case coordination and information sharing.
  • Act as an advocate for clients regarding their clinical status and treatment options.
  • Build effective working relationships with the therapeutic team throughout the case management process.
  • Engage with social services, health, and governmental agencies as needed.
  • Maintain communication with clients and agencies to track progress and ensure satisfaction.
  • Document case information accurately and produce necessary reports.
  • Promote quality and cost-effective interventions and outcomes.
  • Assess and address motivational, physical, educational, and psychosocial issues.
  • Adhere to professional standards as outlined by relevant protocols and regulations.

Qualifications:

  • Demonstrated experience in case management or a related field.
  • Strong knowledge of case management principles.
  • Experience with psychological aspects of care.
  • Effective communication skills, including proficiency in Microsoft Office.
  • Excellent organizational and time management abilities.
  • Familiarity with emerging professional and technical knowledge.
  • Strong problem-solving skills and ability to manage multiple tasks.
  • Compassionate and able to work collaboratively in a team environment.
  • Must be over 21 years old with at least 5 years of experience working with families and children.
  • Possess a reliable vehicle with auto insurance in your name or as a designated driver.
  • Hold a valid Nevada Driver's License.
  • Pass a background check and drug screening.
  • A Bachelor's degree in sociology, psychology, or social work is required.

Schedule:

  • Monday - Friday (9 AM to 5 PM) with some off-site work.
