Key Responsibilities for RN/LPN:

  • Conduct ongoing evaluations of the patient's physical and psychosocial conditions
  • Collaborate with the Interdisciplinary Group (IDG), patient/family, and attending physician to develop a comprehensive care plan
  • Provide education and interventions to effectively manage the patient's pain and other symptoms, while assessing responses to treatment
  • Ensure availability of necessary equipment, supplies, medications, and services
  • Adjust the IDG care plan as needed based on the evolving status and needs of the patient/family
  • Maintain accurate and timely documentation of services provided to patients and families
  • Participate regularly in IDG meetings

Qualifications for RN/LPN:

  • Active, unrestricted RN/LPN license in the State of Michigan
  • A minimum of one (1) year of relevant experience as an RN or LPN is required
  • Preferred: One (1) year of management experience
  • Preferred: One (1) year of experience in home health
  • Current CPR Certification
  • Preferred: Community health and diverse Medical-Surgical experience
  • Reliable transportation with a valid driver's license and current insurance
