Job Summary

We are seeking a highly skilled LPN to join our team as a Medication Care Manager. As a Medication Care Manager, you will be responsible for providing high-quality care and services to our residents by administering medication and treatments in a safe and organized manner.

Key Responsibilities
  • Administer medications and treatments to residents in a safe and organized manner.
  • Review and document medication administration records to ensure accuracy and compliance.
  • Collaborate with the Resident Care Director and Wellness Nurse to ensure medication updates and changes are implemented.
  • Conduct regular audits to ensure medication carts are stocked and organized.
  • Participate in daily Cross Over meetings to discuss resident care and medication administration.
  • Develop and implement Individualized Service Plans (ISPs) and Individualized Care Plans (ICPs) to ensure resident needs are met.
  • Transcribe orders and maintain accurate records.
  • Participate in training and education programs to stay up-to-date on medication administration and resident care best practices.
Requirements
  • LPN, LVN, or state/province specific licensed nurse credential.
  • CPR and First Aid certification.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Ability to maintain confidentiality and handle sensitive information.
  • Ability to work in a fast-paced environment and prioritize tasks effectively.
